In this two-day virtual workshop we will explore nutrition, physical activity and the cancer survivor population across the continuum of cancer care. We will hear from experts about the research surrounding nutrition and physical activity and how to address these issues with cancer patients. Additionally, we will highlight community-clinical linkages that support community partners and health systems working together, and discuss how patient navigation strengthens community-clinical linkages, leading survivors to appropriate support services.
